Criteria
Heywood House will accept young adults from any of the local authorities. We specialise in young adults with profound and complex learning difficulties and additional needs.
Process
1)Discussion with parents / carers, transition team and Heywood Housewith information from “Improving Choices” paperwork.
2)Visit from Heywood House to the current provision for the young adult to assess their needs.
3)Agreement between parents / carers, professionals involved and Heywood House staff that their needs can be met.
4)Application for funding, which may be through The Young Peoples Learning Authority (YPLA), Local Authority Social services and/or Health, by the appropriate professionals.
5)A detailed transition plan agreed with all concerned parties.
6) Implementation of extensive transition period.
