Criteria

Heywood House will accept young adults from any of the local authorities. We specialise in young adults with profound and complex learning difficulties and additional needs.

Process

Family visit to Heywood House.


Discussion with parents / carers, transition team and Heywood House.


Visit from Heywood House to the current provision for the young adult to assess their needs.


Prospective client assessment visit to Heywood House.


Agreement between parents / carers, professionals involved and Heywood House staff that their needs can be met.


Application for funding, which may be through Education, Local Authority Social services and/or Health, by the appropriate professionals. Direct payment or personal budget may also be used.


Once funding is agreed a detailed transition plan agreed with all concerned parties.
